Job Description

The Team Manager is the leader of the Patient Care Team and is responsible for overseeing, evaluating, and coordinating the interdisciplinary team. They ensure continuity of care from admission to discharge or transfer to bereavement, advocate for patients, and coordinate with other social service and health care providers in the community. They manage patient records from intake through discharge or transfer to bereavement.

Patient

Care Services
  • Direct all patient care services for the team, including volunteer and bereavement, ensuring services are integrated components of the interdisciplinary plan of care.
  • Review information on all pending patient admissions, new admissions (assessments and history), and “alive” discharges to confirm eligibility and appropriateness.
  • Collaborate with the team physician and attending physician on palliative care measures for pain and symptom management.
  • Ensure complete and accurate data and documentation are available for recertification decisions and that those decisions are made promptly.
  • Review all imminent “alive” discharges to ensure effective discharge planning.
  • Monitor the type and level of team services provided (staffing, medications, HME, supplies, level of care changes) to ensure appropriateness and responsiveness to patient/family needs.
  • Manage the team schedule and coordinate services 24 hours a day, 7 days a week for all patients on the team.
  • Facilitate team meetings, ensuring interdisciplinary input for updating and revising the plan of care.
  • Participate in the on‑call rotation.
Staff Supervision and Management
  • Interview, select, train, supervise, evaluate, and dismiss team staff in conjunction with the Patient Care Administrator.
  • Ensure staff competence and performance levels through field visits, documentation evaluation, and provision of team‑based services aligned with VITAS values and hospice principles.
  • Act as a resource and mentor for staff on clinical issues, documentation, problem solving, and customer service behavior.
  • Oversee staff and volunteer schedules, scheduling and territory assignments to distribute workload equitably and meet productivity expectations.
  • Review and approve payroll, ensuring accurate time, mileage, and expenses.
Quality Improvement / Regulatory Compliance
  • Monitor clinical records and team activities to ensure accuracy, completeness, and compliance with VITAS policy and regulatory bodies.
  • Develop and implement performance improvement activities to address service issues and challenges.
  • Ensure completion of proactive and patient/family satisfaction surveys and implement quality improvement measures.
  • Assist with surveys and requests from regulatory bodies as directed by the local program.
  • Perform substantive chart reviews to verify quality care delivery.
  • Participate in Outcomes Management and annual program review.
  • Conduct utilization reviews of continuous care and inpatient levels of care for all team patients.
Customer Service / Sales / Marketing
  • Address and resolve problems, grievances, or service failures experienced by patients, families, physicians, or MCOs with team members, vendors, and other VITAS departments.
  • Analyze customer service issues on the team to identify causes and improve performance.
  • Speak personally with patients/families and attending physicians when revocation is considered to seek satisfactory solutions.
  • Regularly visit LTC and contract bed facilities to ensure care plan integration and customer satisfaction.
  • Participate in providing in‑services to customers with other team members, Patient Care Administrators, and program staff.
  • Engage in professional, voluntary, or community service organizations.
Cost Containment
  • Verify/approve admitting IPOC diagnosis, treatments, staffing, and supplies.
  • Monitor utilization of resources for each patient to ensure cost‑effective delivery.
  • Control and be accountable for productivity, labor, and related patient‑care costs versus budget.
  • Approve all bills and invoices related to patient‑care services.
