Physical Therapist

Job Summary

The licensed Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff. Territory will cover Naples to Marco Island, FL.

• Provide PT services prescribed by a physician, which can be safely provided in the home to include:
• Initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points (G333, G336).
• Developing and revising the plan of care in consultation with the physician and other care-team members (G162, G186).
• Providing therapeutic treatments and evaluation of equipment needs as applicable.
• Advising and consulting with the family and other agency personnel as applicable regarding medical equipment, HEP, safety Issues (G143, G188).
• Preparing clinical and progress notes (G187).
• Evaluating outcomes of care.
• Supervising Therapy Assistants and Home Health Aides as appropriate (G190).
• Planning for discharge.

• Will follow all policies and procedures including safeguarding the privacy of protected patient health information and the Compliance Program.
• Participating in in-services and QA programs as requested. (G189).
• All other duties as assigned.

• Graduate of an accredited PT program
• Minimal 1 year experience within Home Health Physical Therapy

• Valid Florida Driver’s License.
• Licensed Physical Therapist in the state of Florida
• Valid CPR certification
