Key Responsibilities

• Provide comprehensive outpatient primary care to an assigned Veteran panel
• Practice within the PACT (team-based) model, collaborating with nursing, pharmacy, and mental health
• Lead evidence-based management of chronic and complex medical conditions
• Emphasize prevention, screening, and whole-health care planning
• Utilize VAs integrated EHR and specialty consult network to coordinate care
• Participate in quality improvement, patient safety, and population health initiative
