Position SummaryThe registered nurse is accountable for the delivery and supervision of safe, quality care that is individualized for assigned patients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning, implementation, evaluation). The RN assesses, treats, and cares for patients across populations and ages as assigned. She/He promotes professional development and communicates the patient’s plan of care and activities with the health care team.
Families and/or significant others are included in the plan of care as appropriate. The RN directs, delegates, and manages care delivered by colleagues and subordinates, and supports family-centered care. The practice aligns with the philosophy of the Division of Nursing, the Mission and Core Values of the Sisters of Providence, and National Patient Safety Standards.
- Possesses knowledge of developmental needs and is competent in the assessment and treatment related to the age of the patient population served.
- Conducts and documents a comprehensive patient assessment upon admission.
- Continually reassesses and applies appropriate advocacy and interventions as indicated.
- Interprets a patient’s self-assessment and behavior with consideration of cognitive, physical, emotional, psychosocial, and maturation factors.
- Actively involves the patient and family in the collaborative plan of care.
- Demonstrates and promotes safe patient care practices, including hand washing, patient identification, communication, infection risk reduction, medication safety and reconciliation, falls prevention, and addressing patient/family concerns.
- Collaborates with patient, family, and health care team to develop, document, implement, and monitor an individualized plan of care based on best practice and evidence-based medicine.
- Provides, documents, and consults to ensure necessary patient and family education in support of the treatment plan.
- Adapts to change and updated patient care practices within an evolving healthcare environment.
- Collaborates in the development of a proactive discharge plan with team members, patient, and family.
- Performs procedures and treatments according to department/unit protocols, guidelines, standards, and the Nurse Practice Act.
- Documents patient care activities legibly and electronically in an accurate, concise, and timely manner.
- Inputs and retrieves information within written and electronic documentation systems.
- Delivers safe, quality care in an efficient, cost-effective manner.
- Supports and assists with orientation of new staff and students.
- Gains necessary training to perform preceptor and charge nurse roles when selected and functioning in those roles.
- Advances professional nursing practice through continuing education, required training, certifications, and participation in department activities.
- Ensures competencies, certifications, safety information, and required courses are completed.
- Identifies concerns and involves colleagues and management to promote a safe patient care environment.
- Engages in positive communication and ongoing development of self and team.
- Promotes open communication, patient- and family-centered care, and healthy collegial relationships.
- Communicates adaptively based on individual patient/family needs.
- Attends or reviews minutes of at least 50% of scheduled staff meetings and remains informed of content.
- Models collaborative practice with other health care professionals.
- Recognizes the impact of cultural diversity on relationships with patients, families, employees, and medical staff.
- R rounds with physicians and the care team to promote continuity of care and patient confidence.
- Monitors patient condition, notes changes, exercises judgment, and takes appropriate action.
- Communicates patient status to colleagues and physicians using SBAR format and supports handoffs with quality and safety tools.
- Utilizes the Providence Early Assessment Team (P.E.A.T.) when indicated by patient changes or concerns.
- Promotes comfort and pain management measures.
- Maintains documentation reflecting assessment, treatment, and re-assessment to meet patient goals.
- Adheres to safety and infection control policies, National Patient Safety Goals, and Emergency Response procedures.
- Complies with hospital standards to ensure regulatory requirements are met (e.g., JCAHO, OSHA, state and federal requirements).
- Participates in departmental and unit-based activities, including Quality Improvement programs and tracer rounds.
- Safeguards all forms of confidential information related to patients, families, staff, and affiliates.
- Adheres to organizational policies regarding Fraud and Abuse, Conflict of Interest, and Code of Conduct.
