Schedule Information
- Shared housing provided at no cost.
- MGB Nantucket Housing Agreement required upon hire if staying in NCH housing.
- MGB FEMA ICS 100 Training must be completed, no exceptions, does not expire.
- N95 fit test required within 365 days to start date and updated annually.
- MGB Color Vision test required within 365 days of start.
- Documentation of MMR vaccination or immunity required.
- Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions.
- TB test screening within 3 months of start date required and updated if >6 month break.
- Health statement signed by MD/DO/NP/PA within 365 days of start date required.
- MGB Criminal Background Check within 45 days prior to start date required.
- MGB Massachusetts CORI background check within 30 days prior to start date required.
- MGB Resume including past 7 years of work experience required for submission.
- Respirator Medical Clearance (RMC) Attestation required.
- MGB References (2 recent, minimum 1 supervisor or above) required for submission.
- MGB Skills Checklist required for submission.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
- MGB Education Verification required.
- MGB SSN Trace required.
- MGB National Sex Offender Check required.
- MGB Exclusion Checks required.
- MGB TB Symptom Survey (if Positive PPD) optional.
