Role Overview
The Referrals Specialist manages all incoming referrals for services, ensuring accurate, prompt processing in compliance with agency and state requirements. The role serves as a key liaison between families, service coordinators, and internal departments to support smooth transitions and timely access to services.
Key Responsibilities- Referral Management – Process referrals accurately and promptly.
- Communication & Coordination – Confirm referral details, conduct customer interviews, collaborate with HR and leadership, and provide status updates.
- Compliance & Documentation – Process referrals per state and agency guidelines, maintain confidential documentation, and support audit readiness.
- Customer Service – Communicate clearly and compassionately, resolve questions or issues professionally.
- Collaboration – Anticipate staffing needs, assist with reporting referral trends to support workforce planning and program development.
- High school diploma or equivalent required;
Associate’s or Bachelor’s degree in Human Services, Healthcare, or related field preferred. - Previous experience in referrals, scheduling, care coordination, or a related administrative role.
- Strong organizational and multitasking skills with attention to detail.
- Excellent verbal and written communication abilities.
- Ability to work independently and as part of a team in a fast‑paced environment.
- Experience in healthcare, developmental disability services, or social services setting.
- Familiarity with state referral processes, Medicaid Waiver services, or case management systems.
- Bilingual skills (English/Spanish) a plus.
- Office‑based with potential for hybrid flexibility.
- Requires frequent communication with families, providers, and state agencies.
