Role Overview

The Referrals Specialist manages all incoming referrals for services, ensuring accurate, prompt processing in compliance with agency and state requirements. The role serves as a key liaison between families, service coordinators, and internal departments to support smooth transitions and timely access to services.

Key Responsibilities
  • Referral Management – Process referrals accurately and promptly.
  • Communication & Coordination – Confirm referral details, conduct customer interviews, collaborate with HR and leadership, and provide status updates.
  • Compliance & Documentation – Process referrals per state and agency guidelines, maintain confidential documentation, and support audit readiness.
  • Customer Service – Communicate clearly and compassionately, resolve questions or issues professionally.
  • Collaboration – Anticipate staffing needs, assist with reporting referral trends to support workforce planning and program development.
Qualifications
  • High school diploma or equivalent required;
    Associate’s or Bachelor’s degree in Human Services, Healthcare, or related field preferred.
  • Previous experience in referrals, scheduling, care coordination, or a related administrative role.
  • Strong organizational and multitasking skills with attention to detail.
  • Excellent verbal and written communication abilities.
  • Ability to work independently and as part of a team in a fast‑paced environment.
Preferred Skills
  • Experience in healthcare, developmental disability services, or social services setting.
  • Familiarity with state referral processes, Medicaid Waiver services, or case management systems.
  • Bilingual skills (English/Spanish) a plus.
Work Environment
  • Office‑based with potential for hybrid flexibility.
  • Requires frequent communication with families, providers, and state agencies.
