RN Case Manager PRN – Grand Strand Medical Center – HCA, Myrtle Beach, South Carolina

Job Summary and Qualifications

We are seeking an RN Case Manager who will coordinate the plan of care for the patient stay, manage length of stay, ensure appropriate resource management, and develop safe discharge plans. You will facilitate the progression and transition of care, perform comprehensive assessments, collaborate with Social Services, and advocate for patient rights.

Responsibilities
  • Coordinate the plan of care for the patient stay, manage length of stay, ensure appropriate resource management, and develop safe discharge plans.
  • Facilitate the progression and transition of care and identify patients at risk for adverse outcomes during transitions.
  • Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients/families, reassess clinical condition as indicated.
  • Ensure post‑acute medical needs and level of care are appropriate in partnership with Social Services.
  • Evaluate progression of care using evidence‑based tools and approved criteria (Inter Qual) throughout the episode of care; address escalation issues.
  • Make referrals to third‑party payers, disease and case‑management programs for recurring patients and those with chronic conditions.
  • Act as liaison through professional communication between physicians, patients/families, hospital staff, and outside agencies; advocate for patient rights, needs, and values.
Qualifications
  • Graduate of an accredited school of nursing:
    Associate’s Degree in Nursing or higher required; BSN highly preferred.
  • Licensed as a Registered Nurse in the state of practice or hold an active Compact/Multi‑State RN license.
  • 1–3 years of clinical, hospital nursing experience required.
  • 2 years of case‑management experience highly preferred.
  • Certification in Case Management or Utilization Review preferred.
  • Inter Qual experience preferred.
