RN Case Manager Emergency (FT) – Carolina Forest

McLeod Health is seeking a RN Case Manager for our Emergency Department in Carolina Forest. The role is full‑time and focuses on coordinating acute care, managing discharge planning, and ensuring high quality, cost‑effective patient outcomes.

• Oversee the continuum of care for ED patients, working collaboratively with physicians, nurses, and allied health professionals.
• Conduct patient assessments, develop individualized care plans, and coordinate services to meet medical necessity.
• Monitor length of stay, resource use, and patient flow, implementing process improvements to reduce delays and optimize reimbursement.
• Advocate for patients/families, ensuring clear communication of the plan of care and supporting discharge planning.
• Assist with utilization management activities, including appropriate level of care determination and billing coordination with third‑party payors.
• Perform additional duties as assigned by the Case Manager Supervisor or Director.

• Registered Nurse license required.
• One to two years of recent acute care or emergency department experience.
• Data entry/computer proficiency.
• Strong communication, critical thinking, and organizational skills.
