As an RN Case Manager, you will deliver exceptional hospice care by providing comprehensive nursing assessments, implementing individualized care plans, and supporting patients and their families with expert guidance and compassionate care.

• Conduct initial and ongoing comprehensive nursing assessments to identify patient needs and develop personalized care plans.
• Implement, evaluate, and adjust care plans in collaboration with the Hospice Interdisciplinary Team (IDT).
• Provide education, supervision, and emotional support to patients, families, and caregivers.
• Ensure high‑quality, compassionate care in accordance with hospice guidelines and best practices.

• Active RN license in South Carolina
• Minimum of two years of nursing experience, preferably in home health or hospice.
• Valid driver’s license, auto insurance, and reliable transportation for field visits.
• Ability to pass background checks, drug screening, and motor vehicle record review.

This role requires a combination of sitting, standing, walking, bending, reaching, and occasional lifting/carrying. Driving, documentation, and patient interaction are also key aspects of the job. Contact Human Resources for a full job description.
