Position: Certified Nursing Aide (CNA) - Hospice

Overview

We are looking for a dedicated candidate to join our team as a Certified Nursing Assistant (CNA) to service our hospice patients in the Grand strand area. The Certified Nursing Assistant (CNA) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality of service delivery. This role includes visiting patients in their homes and Assisted Living Facilities (ALFs) to facilitate visits with providers and specialty services.

Additionally, the CNA is responsible for delivering medications and durable medical equipment (DME) to ensure patients have timely and safe access to essential supplies.

Service Area
  • Grand strand (Conway, Myrtle Beach & Goergetown)
Responsibilities
  • Perform visits in homes and facilities (ALF and ILF's) in their designated service area.
  • You must have reliable transportation as travel is required daily.
  • This is a full-time, salary-based position with 8-hour shifts (8:00 AM – 5:00 PM), Monday–Friday.
  • Work closely with care team members to ensure coordinated patient care and adherence to the care plan.
  • Communicate effectively via phone, email, Microsoft Teams, Athena, in person, and other channels as needed.
  • Provide compassionate personal care to hospice patients in their homes, including bathing, grooming, oral hygiene, dressing, and toileting.
  • Assist patients with safe mobility, transferring, repositioning, and fall-prevention support.
  • Perform light housekeeping tasks related to patient care (changing linens, cleaning care areas, laundering patient items).
  • Monitor and document patient condition changes, including skin integrity, appetite, comfort levels, and safety concerns.
  • Report any significant changes in patient condition to the hospice nurse or clinical team promptly.
  • Support patients with activities of daily living (ADLs) while promoting dignity, comfort, and independence.
  • Provide emotional support and companionship to patients and their families.
  • Assist with meal preparation, feeding, and ensuring adequate hydration based on care plans.
  • Deliver medications to patients’ homes, review instructions, and perform medication reconciliation to ensure accuracy between current medication lists and home records.
  • Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers to ensure timely care.
  • Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.
  • Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.
Qualifications
  • Certified Nursing Aide required. Certifications or licenses must be in good standing.
  • Phlebotomy experience preferred. If not experienced in phlebotomy, candidate must be willing to complete training and demonstrate competency in the skill.
  • Previous experience in public health or home health preferred.
  • Experience with DME delivery or logistics operations preferred.
  • Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
  • Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
  • Ability to read and communicate effectively. Strong written and verbal communication skills.
  • Basic computer knowledge.
  • Must be able to work independently with minimal supervision and direction.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Must be able to work flexible hours and travel between offices, facilities, etc.
  • Must be a licensed driver with an automobile that is insured and in good working condition.
  • Must have a clean driving record and experience operating vehicles in a professional capacity.
