Looking for a compassionate LPN to service our Home Care patients in the Muskegon, MI Area.

Position Overview :

As a Home Health LPN at Trinity Health At Home you'll provide skilled nursing care to the organization's clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse.

Position Highlights:

* Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.

* Maintains a working knowledge of applicable Federal. State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical, and professional behavior.

* Performs specific treatments and administers medications to implement the medical and nursing care plans.

* Apply appropriate preventive, therapeutic, and rehabilitative nursing procedures, and techniques to clients.

Minimum Qualifications:

* Graduate of an accredited or state approved school of practical nursing

* Licensure as a LPN in the State of Michigan

* A minimum of one (1) years of experience in an acute care setting; home care experience preferred.

* Phlebotomy and wound care experience required.

* Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.

* Must possess interpersonal skills sufficiently to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.

* Must have current Driver's license and reliable transportation to and from worksite.
