We are looking for a self-motivated, organized individual passionate about patient care, eager to independently manage cases with confidence and accountability. As a Peritoneal Dialysis Registered Nurse (PD RN), you will educate patients and their care partners on peritoneal dialysis in a clinical setting, enabling them to dialyze from home.

You will be responsible for the ongoing care of each patient to ensure their treatment stays on track.

Dialysis experience is NOT required. Nephrology nursing offers a fresh perspective and skill set for newcomers.

What you can expect:

• Provide comprehensive patient care independently.
  • Assess, diagnose, and develop care plans, considering the whole patient. As a case manager, you will track and organize your patients, taking pride in your ownership and witnessing the impact of your care.
• Empower patients by teaching them to administer their own dialysis and manage their health.
  • Training takes time and effort, but the rewards include transforming patients from feeling sick and afraid to strong, independent individuals engaged in their daily lives. Educating patients about dialysis options expands the program's reach to those who will benefit from PD.
  • Partner with home RN team to provide ongoing dialysis education, supporting the growth of the home program, including educating new in-center hemodialysis patients.
• Lead, coach, and guide using your nursing skills.
  • Work independently while building strong relationships with patients, support systems, and care partners.

Schedule:

• Typically, an 8-hour day, Monday through Friday, with weekends and holidays off.
• Flexible scheduling as you manage your own cases.
• Home visits and on-call shifts vary by program and are compensated accordingly.

Requirements:

• Current RN license in the state of practice.
• Minimum 12 months RN experience.
• Valid driver's license, reliable transportation, and insurance.
• Associates Degree in Nursing (A.D.N) from an accredited school; B.S.N or three-year diploma may be substituted.
• Basic computer skills, proficiency in MS Word and Outlook.

Nice-to-haves:

• Experience in Chronic or Acute Dialysis, Home Hemodialysis, or Peritoneal Dialysis.
• Training, teaching, or healthcare education background.
• Interest in telehealth technologies.
