Full-Time Home Health Physical Therapist Assistant ( PTA )
Join Trinity Health At Home as a licensed Physical Therapist Assistant (PTA) to provide physical therapy services to home care patients in the North Muskegon, Twin Lake, and Holton, MI area. The role supports the organization’s clients under the guidance of a Physical Therapist, following written treatment plans established by physicians and therapists.
Position OverviewAs a Home Care Physical Therapy Assistant you will deliver direct physical therapy treatment to clients, help assess needs for therapeutic devices, and instruct clients and families in device use. You will work independently with minimal direct supervision, maintaining professional relationships with clients and families.
Responsibilities- Provide physical therapy services to clients according to physician orders and therapist plans of care.
- Implement care plans developed by the supervisory Physical Therapist.
- Deliver direct physical therapy treatment to clients as assigned.
- Instruct clients and family members in the use and care of therapeutic appliances and devices, such as wheelchairs, braces, etc.
- Assist in assessing the client’s need for physical therapy devices and interventions.
- Report client responses or changes in condition to physician, PT, RN, or branch management as necessary.
- Licensed PTA within the State of Michigan.
- Graduation from a two‑year college‑level program approved by the American Physical Therapy Association.
- Minimum of six months experience under a qualified Physical Therapist.
- Home health experience preferred.
- Ability to function in a practice environment with minimal direct supervision and maintain professional relationships with clients and families.
- Dedication to maintaining clinical practice skills and adherence to agency policies on an ongoing basis.
- Consistency in demonstrating commitment to the mission, the Organizational Code of Ethics, and compliance program adherence.
- Current driver’s license and reliable transportation to and from work sites.
