- The Physician Assistant (PA) works collaboratively with Life Circles medical providers and the interdisciplinary team (IDT) to deliver high‑quality, participant‑centered care. Acting within the scope of their practice, the PA provides medical care to Life Circles participants across multiple settings, including day centers, home visits, hospitals, nursing homes, and assisted living facilities, ensuring comprehensive support and continuity of care.
- Patient Care & Assessment:
Assess, initiate, and coordinate participant care in collaboration with the medical team, interdisciplinary team (IDT), and external agencies. - Monitor chronic conditions, address acute health concerns, and provide treatment in the clinic, participants’ homes, or care facilities (ALF, SNF, AFC).
- Facilitate advanced care planning discussions with participants and caregivers.
- Educate participants and families on managing chronic conditions and treatment plans.
- Communicate effectively with families and caregivers to support informed decision‑making.
- Participate in daily IDT meetings and contribute to collaborative care planning.
- Coordinate with external partners to support smooth transitions and access to services.
- Support quality improvement initiatives and ensure compliance with organizational and regulatory standards.
- Provide rotating on‑call and weekend support as needed to address urgent care needs.
- Completion of an accredited Physician Assistant program.
- Current, unrestricted Physician Assistant license in Michigan.
- Basic Life Support (BLS) and CPR certification (or willingness to obtain).
- Current DEA registration and Michigan Controlled Substance License.
- Minimum of 1 year of experience with frail, elderly, or long‑term care populations preferred.
