Location: Muscle Shoals
Setting: Long-Term Care (LTC) / Rehabilitation Facility
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care and rehabilitation setting under the supervision of a Registered Nurse (RN) or physician. The LPN supports patient recovery by administering medications, monitoring health conditions, and assisting with daily care to ensure residents receive safe, high-quality care.
Key Responsibilities- Provide direct nursing care to residents in accordance with care plans and physician orders
- Administer medications, injections, and treatments as prescribed
- Monitor and document patient vital signs, symptoms, and overall condition
- Assist residents with activities of daily living (ADLs) including mobility, hygiene, and feeding as needed
- Collaborate with physicians, RNs, therapists, and other healthcare staff to support rehabilitation goals
- Maintain accurate and timely patient records and documentation
- Observe and report changes in patient conditions to the supervising RN or physician
- Ensure compliance with facility policies, safety protocols, and infection control standards
- Provide emotional support and education to residents and their families
- Assist with admissions, discharges, and care transitions
- Active Licensed Practical Nurse (LPN) license in the state of Alabama
- Graduate of an accredited Practical Nursing program
- Current CPR/BLS certification
- Knowledge of long-term care and rehabilitation nursing practices
- Strong communication and patient care skills
- Previous experience in Long-Term Care (LTC) or Rehabilitation settings
- Experience with electronic medical records (EMR) systems
- Ability to work effectively in a fast-paced healthcare environment
