Key Responsibilities:

• Conduct comprehensive assessments of patients' speech, language, and swallowing abilities.
• Develop and implement individualized treatment plans based on assessment findings.
• Provide therapy sessions in patients' homes, utilizing evidence-based practices.
• Educate patients and their families on techniques to improve communication and swallowing.
• Monitor and document patients' progress, adjusting treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure coordinated care.
• Maintain accurate and timely patient records in compliance with regulatory standards.
• Participate in team meetings and continuing education opportunities.

Active Speech Therapy License (Utah)

• Valid Driver's license
• Reliable transportation
