LVN/LPN Weekends Sat/Sun (Healthcare)

Responsibilities
  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines governing the long-term care facility.
  • Ensure that all nursing personnel comply with written policies and procedures established by the facility; periodically review policies, procedure manuals, and job descriptions; recommend revisions as needed.
  • Plan shifts’ services, programs, and activities with assigned nursing staff and support personnel.
  • Ensure Nursing Service Procedures Manual is current and reflects day-to-day nursing procedures; ensure compliance with the procedures.
  • Provide written and oral reports/recommendations concerning shift activities; cooperate with other resident services to maintain the resident’s total regimen of care.
  • Ensure all nursing service personnel are in compliance with their job descriptions; participate in the facility’s quality assurance program for the nursing service department.
  • Participate in facility surveys (inspections) conducted by authorized government agencies as requested; review and update the resident’s discharge plan as required; assist in planning the nursing services portion of the discharge plan.
  • Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required.
  • Admit, transfer, and discharge residents as required; complete accident/incident reports; write resident charge slips; maintain Daily Census Report; submit to the Business Office as required.
  • Perform administrative duties including completing medical forms, reports, evaluations, studies, charting, etc.; protect resident information and report suspected violations per facility policy.
  • Charting and Documentation: complete and file required records upon admission, transfer, and discharge; encourage physicians to review and sign orders; transcribe orders to charts; document care provided and resident response; process accident/incident reports; communicate changes to diet orders and coordinate with Food Services.
  • Drug Administration: prepare and administer medications as ordered; verify resident identity; maintain narcotic records; dispose of drugs per procedures; ensure stock levels and orders meet nursing needs.
  • Personnel Functions: participate in staffing decisions; communicate staffing needs; lead nursing personnel; perform rounds to ensure standards are met; ensure CNA training; maintain policy compliance; foster teamwork and a warm, calm unit environment; handle complaints and disciplinary actions per policy.
  • Cooperate with inter-departmental staff to meet resident needs; maintain warm, personal care environment; document and report on issues as required.
  • Cooperate with residents and families; coordinate transportation for discharges as necessary; provide notifications of changes in resident condition and post-mortem procedures as required.
  • Safety and Sanitation: monitor safety procedures; ensure PPE availability and proper hand hygiene; participate in infection control and fire safety drills; follow procedures for hazardous materials; maintain clean and sanitary work areas.
  • Equipment and Supply Functions: oversee inventory and ordering; ensure safe operation of nursing service equipment; train personnel on equipment use; minimize waste.
  • Care Plan and Assessment: review and update care plans; ensure CNAs follow the care plan; assist in planning and revising MDS, RAPs, and triggers.
  • Budget and Planning: report suspected fraud related to billing and cost reports as appropriate.
  • Miscellaneous: provide data to Quality Assurance & Assessment Committee as requested.
Qualifications
  • Education:

    Nursing degree from an accredited college or university, or graduate of an approved LPN/LVN program.
  • Licenses:
    Current, unencumbered, active LPN/LVN license in the state; current CPR certification; maintain required continuing education/licensing; remain in good standing with the State Board of Nursing.
