JOB SUMMARYThis position is filled within a multidisciplinary health care team. Medical social workers provide psychosocial interventions to patients and their families across departments. Responsibilities include assessment of social needs related to health status, development of treatment plans, advocacy within the organization and community, counseling, referrals, practical assistance, and coordination of care. The role also involves consultation, orientation, and training to health care staff on biopsychosocial, cultural, and economic issues, and advocacy for social work values within Stanford Medicine and the broader community.
Licensed Clinical Social Worker: Licensed Clinical Social Worker represents the second, fully-qualified level of professional social work for California-licensed professionals (LCSW). Positions at this level involve substantial professional responsibility and independence of action.
Social Work Clinician: Social Work Clinician is the entry-level position for recent graduates or those not yet licensed as LCSW in California.

Must perform duties in accordance with hospital policies and procedures, including Service Standards and Code of Conduct.
Licensed Clinical Social Worker:
- May provide individual/group clinical supervision to MSWs and student interns, as assigned.
- Prepares and maintains electronic medical records in accordance with department and/or health system policies.
- Provides written and oral reports and hand-offs.
- Conducts psychosocial assessment of patients/families upon admission, annually, and/or as clinically indicated to evaluate social, emotional, and physical needs; identifies and addresses barriers to care.
- Collaborates with interdisciplinary health care teams to devise, develop, and implement patient/family psychosocial treatment plans, coordinating referrals to specialty and social services including entitlement programs and other care delivery systems, and recording progress notes and continuity of care.
- Helps patients/families cope with environmental and psychological issues of illness.
- Manages independently assigned caseload of varying clinical acuity.
- Engages in case finding for patients/families with potential psychosocial needs; attends team conferences and participates in interdisciplinary rounds as needed.
- Provides supportive counseling, grief counseling, crisis intervention, and other therapeutic services to assist patients and families in facing challenges such as illness, medical information, and changes in functioning and care needs.
- Provides psychoeducation to patients and families regarding adaptation to the diagnosis, illness, treatment, and prognosis; assists in transition from youth to adult services.
- Participates in quality assurance and community improvement related to social work services and contributes to policies, procedures, or guidelines.
Social Work Clinician:

- Conducts psychosocial assessment of patients/families upon admission, annually, and/or as clinically indicated to evaluate social, emotional, and physical needs; identifies and addresses barriers to care.
