After‑hours registered nurses support patient needs during the evening on weekdays (Mon‑Thu 5:00 PM–8:30 AM) and a continuous weekend shift (Fri 5:00 PM–Mon 8:30 AM). This role follows a rotating schedule: one week on, followed by one full week off.
The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities after normal business hours.
Job Responsibilities- Completes an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses.
- Provides professional nursing care by utilizing all elements of the nursing process and utilizes health assessment data to determine nursing diagnosis.
- Assesses and evaluates patient status through writing and initiating plan of care, re‑evaluating patient and family/caregiver needs after hours when needed, and participates in revising the plan of care as necessary.
- Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
- Assists the patient with activities of daily living and facilitates the patient's efforts toward self‑sufficiency and optional comfort care.
- Supports the Case Managers and assumes responsibility to coordinate patient care for the agency after hours.
- Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
- Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
- Teaches the patient and family/caregiver self‑care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
- Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
- Actively participates in quality assessment performance improvement teams and activities if needed.
- Performs additional job duties as assigned.
- Bachelor's or Associate's degree in nursing or related fields, required.
- Maintains an active nursing license that is in good standing in the state performing job duties.
- Maintains an active CPR certification.
- At least 2–5 years of experience in healthcare management, hospice, or palliative care and/or training in end‑of‑life care, preferred.
- At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred.
- Knowledge of hospice care and the services provided to patient and family/caregiver through an interdisciplinary group.
- Proficient in MS Office applications and ability to learn department and job‑specific software systems.
- Demonstrate organizational skills.
- Demonstrate effective verbal and written communication skills.
- Demonstrate analytical skills when problem‑solving.
- Demonstrate high attention to detail and a high degree of accuracy.
