The Substance Use Disorder (SUD) Case Manager is responsible for providing comprehensive case management services to clients with substance use disorders. This role includes conducting assessments, coordinating services, developing treatment and recovery plans, monitoring progress, and advocating for clients. The SUD Case Manager will work closely with clients, families, healthcare providers, and external partners to support long-term recovery and well-being.

Key Responsibilities Ongoing Assessments
  • Conduct Social Determinants of Needs Assessments and SUD Case Management Assessments within the first 5 days of treatment.
  • Complete follow-up assessments monthly to evaluate client progress.
  • Ensure that all assessments are uploaded and attached to the client’s therapy notes chart.
  • Use these assessments to develop and review the client’s Individualized Treatment Plan (ITP).
Recovery Capital Scale
  • Complete the Recovery Capital Scale every 90 days to assess recovery progress.
  • Ensure results are uploaded to the client’s chart.
  • Use results to guide and review the client’s Recovery Plan.
Treatment Planning
  • Develop and update treatment plans within the first 5 days after the client’s ASAM assessment.
  • Update treatment plans monthly, ensuring alignment with the ITP template and goals based on the SUD Case Management Assessment.
  • Coordinate treatment planning with client feedback and assessment results.
Recovery Plan Development
  • Review and update Recovery Plans weekly during Monday group sessions.
  • Ensure clients have access to necessary resources to complete their goals.
Coordinating Services
  • Ensure clients are meeting group requirements, individual sessions, UDS, and nursing services.
  • Act as a liaison between clients and various services, including medical, mental health, housing, legal, and employment.
  • Provide referrals as needed to support client recovery.
Client Education
  • Educate clients and their families on addiction, recovery, and healthy coping mechanisms.
  • Promote awareness of recovery resources to enhance long-term wellness.
Monitoring Progress
  • Conduct daily check-ins with clients to track treatment progress.
  • Review UDS results and ITP goals with clients regularly to ensure progress.
Advocacy
  • Advocate for client access to necessary services and resources.
  • Collaborate with legal systems, insurance providers, healthcare teams, and probation/parole officers as needed (bi-weekly check-ins with PO if applicable).
Documentation
  • Maintain accurate and timely records of client assessments, progress, and treatment plans.
  • Ensure compliance with all regulatory requirements and confidentiality standards.
Relapse Maintenance
  • Begin relapse maintenance planning when Social Determinants of Needs are met, and the client’s Recovery Capital Score has increased by 35 points.
  • Use the Recovery Plan Prompts to facilitate in-depth relapse planning.
  • Conduct follow-up sessions on living and balance recovery maintenance.
Specific Duties Clinical Oversight
  • Monitor the effectiveness of case management and treatment services.
  • Daily Engagement with client.
  • Make recommendations for improvement as necessary to enhance client outcomes.
Policy Development
  • Follow and apply all agency policies and procedures related to SUD case management and treatment planning.
Supervision of Client Progress
  • Collaborate with clinical staff to ensure alignment between treatment goals, client assessments, and progress reviews.
Compliance and Accreditation
  • Ensure compliance with all relevant laws, regulations, and accreditation standards.
Service Coordination
  • Act as a key liaison between clients and various service providers to promote a holistic approach to recovery.
Qualifications
  • High school Diploma or Equivalent Required
  • Bachelor’s degree in Social Work, Counseling, or a related field (Preferred).
  • Certification or experience in substance use disorder treatment and case management (preferred).
  • Knowledge of ASAM criteria, treatment planning, and recovery capital assessments.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively in a multidisciplinary environment.
Working Conditions
