Job Title: Licensed Master Social Worker (LMSW) – Hybrid Opportunities

Job Type: Full-Time Hybrid (Office-based with remote flexibility)

Location: Mount Vernon, NY

Job Description:
The Licensed Master Social Worker (LMSW) is responsible for performing daily social work operations and delivering evidence-based behavioral health services.

Key responsibilities include:

• Conducting assessments of patients referred by Primary Care Providers (PCPs) using evidence-based assessment tools.
• Providing treatment for behavioral health conditions using evidence-based approaches, including CBT, Motivational Interviewing, crisis intervention, Interpersonal Psychotherapy, and behavioral activation.
• Making treatment recommendations during collaborative care team meetings.
• Monitoring treatment progress through care planning and coordination with clinical teams and health homes.
• Providing crisis intervention services and performing risk assessments as needed.
• Engaging clients with limited utilization of behavioral health services through outreach efforts.
• Documenting clinical services accurately and in a timely manner.

Requirements:

• Current LMSW licensure.
• Master of Social Work (MSW) degree from an accredited institution.
• Ability to utilize behavioral health screening tools and conduct biopsychosocial assessments.
• Proficiency in using DSM-V diagnostic criteria to diagnose and treat behavioral health conditions.
• Experience assessing risk and collaborating with psychiatric and clinical teams on risk management.
• Ability to utilize electronic health records for documentation and review of relevant health data.
• Strong communication skills to clearly convey clinical information to integrated care teams, including PCPs, NP/MDs, and Health Home Care Management staff.
• Bilingual Spanish/English strongly preferred.
