Position: FAMILY MEDICINE PHYSICIAN
Location: Mt Rainier

We are currently looking for a caring, ambitious, energetic individual to provide primary and telemedical care for patients transitioning out of prison. We value quality of care, and achieving healthcare metrics, reducing hospitalizations and re-hospitalizations, decreased emergency department utilization. MAJOR

DUTIES & RESPONSIBILITIES:
  • Provides patients with medical assessments and treatments pertaining to physical and mental health.
  • Orders and interprets medical labs and diagnostic imaging for patients.
  • Refers patients to appropriate specialists as needed.
  • Prescribes pharmaceuticals and treatments for assessed medical conditions.
  • Performs minor procedures as qualified.
  • Provides accurate, timely, and complete documentation in patient records following coding standards.
  • Displays sensitivity to the populations served.
  • Contributes professional medical expertise to other medical and mental health professionals and clinical support staff.
  • Provides oversight of support staff with respect to individual patient management.
  • Participates in team huddles to review the schedule at the start of the clinic day.
  • Participates in process and quality improvement workflows.
  • Maintains punctual, regular, and predictable attendance.
  • Maintains availability during scheduled on-call hours.
  • Attends required meetings and in-services, and participates in committees as requested.
  • Performs other duties as assigned.
  • Maintain and complete patients' health records or charts legibly, concisely, and close them within 48 hours and/or by the end of the serviced week;
  • Work as an integral part of the clinical team;
  • Participate in the on-call/Saturday rotation schedule as needed;
  • Be flexible to work between different sites;
  • Comply with the regulations and policies required of a Federally Qualified Health Center;
  • Complete administrative assignments as requested by Medical Director and/or clinic;
  • Report to the Medical Director;
  • Perform other duties as assigned to support Golden Health Initiative’s Mission, Vision, and Values.
Education And/or

Experience:
  • Doctoral degree (MD/DO) or equivalent from an accredited school of medicine is required.
  • Completion of Family Practice Residency with at least two years of related experience and/or training preferred.
  • Experience working with underserved populations is a plus.
Certificates, Licenses & Registrations:
  • This position requires an active and unrestricted license to practice medicine in the State of Arizona.
  • Must have an active DEA license.
  • BLS certification required.
  • Must be able to provide all necessary documentation for credentialing.
