This position will serve the Knox, Blount, Wilson Counties in TN.
Field-Based Role: Associates operate primarily in the field, traveling to client sites as required, with occasional office attendance for meetings or training. Candidates not within a reasonable commuting distance from the posting location(s) will not be considered unless an accommodation is granted as required by law.
LTSS Service CoordinatorResponsible for conducting service coordination functions for a defined caseload of individuals in specialized programs. Collaborates with the person supported to facilitate the Person-Centered Planning process, including assessments, development of a comprehensive Person-Centered Support Plan (PCSP), engagement of the member’s circle of support, and overall management of the individual’s physical health (PH)/behavioral health (BH)/LTSS needs.
How You Will Make An Impact- Perform face-to-face program assessments using pre-defined tools and motivational interviewing techniques for evaluation, coordination, and management of an individual’s waiver (LTSS/IDD), BH, or PH needs.
- Identify members with potential clinical health care needs and coordinate their cases with the interdisciplinary team.
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, or disabilities for cost-effective utilization of long-term services and supports.
- Document short- and long-term service and support goals in collaboration with the member’s chosen care team.
- Identify members that would benefit from alternative levels of service or other waiver programs.
- Serve as mentor, subject matter expert, or preceptor for new staff and participate in process improvement initiatives.
- Submit utilization/authorization requests to utilization management with supporting documentation aligned with the individual’s care plan.
- Report critical incidents to appropriate internal and external parties such as state and county agencies (Adult Protective Services, Law Enforcement).
- Assist and participate in appeal or fair hearings, member grievances, appeals, and state audits.
- BA/BS degree and a minimum of 2 years of experience working with a social work agency, or equivalent combination of education and experience.
- Specific education, years, and type of experience may be required based on state law and contract requirements.
- BA/BS degree in a health care related field.
- Willingness to travel to worksite and other locations as necessary.
