FLSA STATUS:
Non-exempt. Overtime pursuant to Collective Bargaining Agreement or Human Resources and Labor Relations Policy
EMPLOYMENT RELATIONSHIP:
Just cause subject to probationary period
CURRENT HOURS AND STARTING TIME:
Full-time (37.5 hours per week) position. The starting time for this position is currently 8:00 a.m.
GENERAL
SUMMARY:
Under direction, facilitates prompt and effective coordination of services regarding the health of our persons served, including, but not limited to, their medical, case management, and medication needs within Macomb County Community Mental Health (MCCMH) programs. Provides direct services to persons served and their families. Provides specialized health care assessments and screenings. Implements program activities. Participates in research and program evaluation to improve coordination of health and medication programming.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Promotes the integration of care through the coordination of physical and mental health needs for people served within outpatient and case management programs.
Implements, coordinates, monitors, and evaluates services required to meet the mental and physical health needs of persons served in conjunction with goals established with a Person-Centered Plan.
Ensures coordination of health care services across a variety of disciplines with other service providers.
Provides direct clinical nursing services to persons served including, but not limited to, administering injections and dispensing medication, monitoring ongoing health care needs, case management, patient care coordination, and transportation of persons served as needed.
Completes initial and ongoing assessments, which include emergency and routine health care assessments.
Provides crisis intervention techniques to stabilize situations as needed.
Establishes and implements treatment plans, utilizing a Person-Centered Plan or a Family-Centered Practice approach.
Completes and maintains record keeping requirements, including case documentation.
Participates in case consultation to confirm diagnosis, makes clinical recommendations, provide appropriate form of treatment, and link the person served and/or family to the appropriate program.
Maintains inventory of agency medications at assigned site(s).
Requests appropriate authorizations for services provided, monitors utilization of services, and requests re-authorization of continuation of service as medically necessary.
Supports MCCMH integrated health interventions by interacting with managed care providers, insurance providers, private physicians, hospital services, and other entities pertinent to the health of MCCMH people served.
Maintains ongoing communication with members of agency staff and contract psychiatric staff.
Develops and shares current information about appropriate resources for people served.
Provides training and educational activities to staff, clients, families, and MCCMH service providers.
Attends and participates in clinic and committee meetings, conferences, and in-service training.
Participates in staff development, planning, agency goal setting, and quality improvement.
Maintains and updates clinical knowledge through readings, in-service training, and other related activities.
Operates an automobile to perform assigned job functions.
Performs related duties as assigned.
QUALIFICATIONS:
Required
Education and Experience
* Associate's degree from an accredited college or university in Nursing
Preferred
Education and Experience
* Bachelor's Degree from an accredited college or university in Nursing
* A minimum of six (6) months experience working with the mentally ill population
Required Licenses or Certifications
* Current State of Michigan Registered Nurse license
* Possession of a valid Michigan driver's license and operable, insured automobile for authorized travel
* If hired within the Assertive Community Treatment (ACT) program, must possess and maintain a current Michigan Chauffeur's license within two (2) weeks from date of hire
