Case Manager - Macomb

This position does require working in the community and performing home visits.

Bachelor’s degree in a human services field from an accredited institution required.

LLBSW, LBSW, LLMSW, LMSW, LMFT preferred.

• Coordinate all services necessary to maximize consumer recovery and independence for individuals with mental illness.
• Provide clinical and program consultation to direct care staff in a residential or clinical setting.
• Ensure case management compliance with agency standards and the practice model.
• Assist consumer to improve quality of life and maintain the highest possible ability to recover and function within the community.
• Implement strategies for obtaining services and supports that are goal-oriented and individualized.
• Ensure consumer involvement in the Person Centered Planning Process (PCP).
• Facilitate the implementation of Individual Plan of Service.
• Perform comprehensive, ongoing psychosocial assessments.
• Participate in legal proceedings including petitioning and testifying in clinical emergencies.
• Provide community based services.
• Facilitate the coordination of consumer intakes, discharges, and transfers.
• Communicate with all service providers in Treatment Plan.
• Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
• Coordinate and monitor activities such as job, school, volunteerism, and family caretaking.
• Provide referral to appropriate community resources.

• Bachelor’s degree in a human services field from an accredited institution required.
• LLBSW, LBSW, LLMSW, LMSW, LMFT preferred.
• Prior case management experience in a mental health or rehabilitation setting preferred.
• Valid Michigan Driver’s License.
• Driver's License.

• Starting Pay: $21.50 - 27.00 (depending on licensure and education)
