Summary: Processes and maintains patient medical records including filing, retrieving and distributing medical documents and reports according to established priorities and procedures in a medical records department.

• Maintains record filing and electronic systems in accordance with established procedures and regulations.
• Performs concurrent scanning rounding on hospital units daily.
• Monitors, reconciles, and audits paper based and electronic medical records. Prepares and batches, indexes, analyzes for quality and modifies medical records for the purpose of maintaining a reliable electronic health record.
• Identifies scanning and indexing errors with timely and accurate correction/modification.
• Retrieves, delivers and coordinates the delivery of patient medical records for release of information requests, research requests and provider requests and ensures they arrive in a timely manner.
• Analyzes all bedded, emergency room, outpatient, and ambulatory surgery for chart completion to include provider assistance as necessary.
• Analyzes and monitors record completion activities of suspended physicians and generates automated incomplete record notices to physicians.
• Releases approved documents as the legal medical record.

• Education Requirements: High School diploma or GED
• Licensure/Certification Requirements: No licensure or certification required.
• Professional Experience Requirements: One (1) year of clerical experience or an equivalent combination of education, training and experience.
• Knowledge/Skills/and Abilities Requirements: Demonstrated ability to successfully manage multiple tasks simultaneously.
