Job Description

Registered Nurse - Hospice

Reports to:

Assistant Manager

Responsibilities
  • Performs patient assessments:
    Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals
  • Develops the plan of care for the patient and family
  • Maintains visit productivity standards
  • Demonstrates knowledge of insurance reimbursement
  • Completes discharge assessments:
    Performs laptop transfer of data twice daily; completes admission assessment and transfers within 24 hours; completes re-visit documentation and transfers
  • Uses age-appropriate teaching methods for patients and families:
    Evaluates teaching plan and revises as necessary; documents patient education tools used
  • Assumes responsibility for case management:
    Develops/implements/evaluates plan of care based on evidence-based knowledge; revises care plan as indicated; prioritizes care needs; demonstrates knowledge of hospice regulations and insurance reimbursement guidelines; shows evidence of interdisciplinary communication; performs HHA supervisions; follows scheduling process; communicates progress toward goals with IDT; evaluates and revises the plan of care as needed; prioritizes schedule to accommodate patient’s needs; documents case management activities;

    demonstrates knowledge of pharmacology and safe medication management; routinely reconciles medications and maintains current medication orders
  • Maintains safe environment for patients and co-workers:
    Follows infection control measures and reports infections; demonstrates knowledge of pharmacology and safe medication management; reconciles medications; maintains current medication orders
  • Attends monthly staff meetings:
    Participates in PI meetings and activities/committees as requested; accepts other assignments as requested
  • Participates in Performance Improvement Program:
    Reviews health records and gathers data to support PI activities; attends UR meetings as requested; provides orientation/mentorship to new employees; cross-trains to learn new skills and demonstrates willingness to teach others
  • Assists with growing agency business:
    Provides orientation to new employees; accepts other assignments as requested; cross trains to learn new skills
Qualifications Required
  • Current NJ RN License
  • BLS
  • Associates Degree in Nursing
Preferred
  • Minimum of one-year experience in acute, subacute or home/hospice care setting
  • Experience using an EMR, excellent critical thinking and communication skills
  • BSN
