RN - Case Management

The In-Patient Case Manager coordinates patient care throughout the acute care hospitalization. This role involves managing all aspects of the discharge planning and transition process, along with performing utilization management and quality screenings.

• Coordinate patients through the acute care hospitalization.
• Manage discharge planning and transition processes.
• Conduct utilization management and quality screenings.
• Ensure compliance with in-patient case management requirements.

• 2 years of acute care experience strongly preferred.
• Case Management and Managed Care experience.
• BSN required.
• Active RN license required.
• CCMC certification preferred.
