• Coordinate and maintain the Minimum Data Set (MDS) process for all residents, ensuring accurate and timely reporting
• Develop and implement care plans that meet the unique needs of each resident, in collaboration with interdisciplinary team members
• Conduct MDS assessments, evaluations, and progress notes, ensuring compliance with regulatory requirements
• Collaborate with healthcare professionals to ensure smooth transitions and coordinate care for residents
• Provide education and training to staff on MDS processes and regulations
• Maintain accurate and up-to-date records and reports, adhering to confidentiality and HIPAA guidelines

• 1 Year experience as an MDS
• Current RN license in the state of New Jersey
• Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities
• Excellent communication and interpersonal skills, with the ability to work collaboratively with a diverse team
• Strong attention to detail, with a focus on accuracy and confidentiality
