Visiting Nurse Services; Homecare Intern

Position: Visiting Nurse Services (Homecare) Intern
Responsibilities Include:

* Interact with Atlantic Visiting Nurse team members in an office and community setting

* Working hand-in-hand with our visiting nurses to see patients in their home environment and help them towards their individual health goals

* Working with our specialty programs, including Remote Patient Monitoring, Virtual Care, and Wound Program, which provide skilled consultations; these programs aim to reduce the risk of patients being re-hospitalized, while providing an elevated level of assessment and collaboration

* Working with our office team members, who support the patient experience and provide daily oversight to workplace operations

* Includes positive collaboration with an interdisciplinary team to report changes in the patient's condition and progress towards goals
