Atlantic Visiting Nurse – Hospice Registered Nurse
Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full‑time hospice Registered Nurse to work 9 AM to 5 PM in the Essex County area, including weekends and on‑call as required.
Responsibilities- Perform patient assessments and develop the plan of care for the patient and family.
- Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
- Complete admission, discharge, and re‑visit assessments, transferring data twice daily.
- Use age‑appropriate teaching methods, evaluating the teaching plan and documenting tools used.
- Assume responsibility for case management: develop, implement, and evaluate the plan of care based on evidence‑based knowledge, revise the plan as indicated, and prioritize care needs.
- Show knowledge of hospice regulations, insurance reimbursement guidelines, and interdisciplinary communication.
- Perform HHA supervisions per standards, ensure appropriate utilization of services, and follow scheduling process.
- Document case management activities, maintain current medication orders, and routinely reconcile medications.
- Attend monthly staff meetings, PI meetings, and IDT meetings every two weeks, communicating progress toward goals.
- Participate in performance improvement program: review health records, gather data to support PI activities, attend UR meetings, provide orientation/mentorship to new employees, cross‑train to learn new skills.
- Graduate of an accredited Nursing Program – Associate’s Degree.
- Current state licensure as a Registered Nurse.
- BLS certification.
- Preferred:
Bachelor’s degree in nursing (ASNs must complete BSN within five years of start date), Master’s degree in nursing or related field, 1+ year of nursing or relevant experience, ACLS certification.
Entry level starts at $43.00/hour with mileage reimbursement ($0.70 per mile). Internal referral bonus $5,000.
