Job Description

The Social Worker (MSW) provides comprehensive psychosocial assessment, intervention, and support to patients and families to promote well-being, continuity of care, and optimal patient outcomes. This role focuses on addressing social drivers of health (SDOH), facilitating safe and effective transitions of care, and supporting patients through illness, adjustment, and life changes. The Social Worker collaborates with interdisciplinary teams and community partners to ensure patient needs are met through coordinated, patient-centered, and equitable care delivery.

This role is specific to the outpatient Prenatal clinic.

This position has no direct reports.

• Master's degree in social work (MSW) from an accredited program required.
• Prior hospital-based or medical social work experience strongly preferred.

• Active New Jersey LSW or LCSW required.

• Spanish speaking preferred
• Excellent communication, negotiation, and conflict‑resolution skills.
• Strong understanding of social drivers of health (SDOH) and their impact on patient outcomes.
• Proficiency in electronic health records and data systems (Epic preferred).
• Ability to collaborate effectively in a fast‑paced, interdisciplinary healthcare environment.
