Overview
An exciting opportunity has arisen to join the District Nursing Service in South East and Central Northumberland for a Community Staff Nurse. We are looking for enthusiastic staff nurses to join our District Nursing team based at Wellway GP Surgery, Lynemouth. This is a fantastic opportunity to become a team member in an exciting service committed to providing patient‑centred care to patients in their own homes, residential care settings and in clinics.
We are keen to provide our staff with development opportunities to enhance their skills, knowledge and career progression within the District Nursing Service, while delivering high quality and safe service in line with the Trust Values. Responsibilities- Work autonomously and as part of a team, reporting directly to the line manager and community nursing team lead, assessing, planning, implementing and evaluating care for a diverse group of patients with a wide range of conditions and health needs.
- Develop and deliver programmes of health and social care.
- Deliver care and support to patients with a terminal illness and their families, including the provision of emotional and bereavement support.
- Perform skilled nursing care, including assessment, wound management, palliative care and catheterisation.
- Perform diagnostic tests such as Ankle Brachial Pressure Index using Doppler and interpret the results to decide further courses of action.
- Perform procedures that require fine dexterity and skill such as venepuncture, drug administration by injection and removal of sutures.
- Interpret results and communicate them to patients (e.g., blood glucose estimation), explaining aspects of care to patients and carers and allowing opportunities for questions and concerns.
- Maintain and promote high standards of professional practice, sharing approaches and experience with others.
- Maintain a duty of care to patients, carers and colleagues through personal and professional accountability.
- Proactively plan patient care and provide vital support to other locality teams during times of staffing shortages and operational pressure.
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state‑of‑the‑art Northumbria Specialist Emergency Care Hospital – the first of its kind in England. We also care for people in their homes and provide services from local health centres. Our goal is to give people greater choice and control over their care, helping them to live independently at home and avoid hospital admission where appropriate.
