• Review ER monitors through the EPIC electronic medical record system to identify patients at high risk for sepsis.
• Communicate findings promptly to the ER team and facilitate the implementation of sepsis protocols.
• Collaborate with the ER team to ensure appropriate tests, interventions, and follow-ups are conducted for high-risk patients.
• Conduct thorough follow-up assessments to monitor patient progress and ensure adherence to sepsis management plans.
• Serve as a resource and provide education to healthcare staff on sepsis identification, assessment, and management.
• Participate in quality improvement initiatives related to sepsis care and outcomes.
• Maintain accurate and detailed documentation of assessments, interventions, and outcomes in compliance with hospital policies and regulatory guidelines.
• Place protocol orders at the direction of the primary physician.

• Education, Certification, and/or Licensure: Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
• Obtain certification in Basic Life Support within 30 days of hire date.

• Experience: Three (3) years of nursing experience.

• Bachelor of Science in Nursing (BSN).
• Care coordination experience.

• Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping are necessary body movements utilized in performing duties through the work shift.
• Ability to sit for extended periods of time.

• Standard call center office environment.
• Will be available for incoming calls and other tasks 24 hours a day and 7 days a week.
