Responsibilities- Review ER monitors through the EPIC electronic medical record system to identify patients at high risk for sepsis.
- Communicate findings promptly to the ER team and facilitate the implementation of sepsis protocols.
- Collaborate with the ER team to ensure appropriate tests, interventions, and follow-ups are conducted for high-risk patients.
- Conduct thorough follow-up assessments to monitor patient progress and ensure adherence to sepsis management plans.
- Serve as a resource and provide education to healthcare staff on sepsis identification, assessment, and management.
- Participate in quality improvement initiatives related to sepsis care and outcomes.
- Maintain accurate and detailed documentation of assessments, interventions, and outcomes in compliance with hospital policies and regulatory guidelines.
- Place protocol orders at the direction of the primary physician.
- Education, Certification, and/or Licensure: Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
- Obtain certification in Basic Life Support within 30 days of hire date.
- Experience: Three (3) years of nursing experience.
- Bachelor of Science in Nursing (BSN).
- Care coordination experience.
- Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping are necessary body movements utilized in performing duties through the work shift.
- Ability to sit for extended periods of time.
- Standard call center office environment.
- Will be available for incoming calls and other tasks 24 hours a day and 7 days a week.
