Description
Easterseals PORT Health (ESPH) is seeking a Licensed Mental Health Therapist to join our Assertive Community Treatment (ACT) Team. This role supports adults with severe mental health and substance use needs by providing therapy in community-based settings, including clients’ homes and neighborhoods.
Your Role in Our MissionAs a Therapist on our ACT Team, you will work with a multidisciplinary team to bring treatment, stability, and hope to clients in real-world settings. What You’ll Experience- Community Impact:
Provide therapy and support in real-world settings to help clients regain independence and stability. - Collaborative Care:
Work daily with psychiatrists, nurses, co-occurring disorder specialists, peer support specialists, and other behavioral health professionals. - Advocacy & Integrity:
Champion client choice and dignity with trauma-informed, person-centered care. - Professional Growth:
Receive up to $65/week toward licensure supervision, plus ongoing training, mentorship, and career development.

- Annual Salary: $52,000 to $65,000 (equivalent to $25.00–$31.25/hour), based on licensure & experience

- Licensure supervision funding (up to $65/week)
- Provide individual and group therapy to 4–5 adult clients per day in the community
- Create individualized treatment plans aligned with ACT best practices
- Participate in daily multidisciplinary team meetings
- Collaborate with families, community partners, and referring agencies
- Maintain timely, accurate documentation in Welligent EMR
- Participate in the rotating on-call crisis schedule as part of ACT model fidelity
- Uphold ethical standards, trauma-informed care principles, and client-centered practice
- Licensure: LCSW/A, LCMHC/A, LMFT/A (provisional or full NC licensure required)
- Experience:
Community-based work preferred; comfortable meeting clients where they are - Requirements:
Valid driver’s license, reliable transportation, and passion for serving vulnerable adults
