Overview
Our community nursing team provides care from 8 am to 8 pm, covering all 7 days of the week including weekends and bank holidays. Team members work part‑time or full‑time on a shift system with early and late shifts. The trust offers an excellent flexible working policy and supports mobile work – you will be equipped with a laptop and smartphone so you can access patient information in real time and complete documentation while on call.
Key Responsibilities- Carry out nursing care in patients’ homes, delivering clinical care and supporting self‑management.
- Record and update patient information accurately and promptly using mobile devices.
- Make clinical decisions with confidence and seek guidance when needed.
- Working in partnership with the wider community team to meet the needs of the service.
Skills & Qualifications
- Current registration with the Nursing and Midwifery Council (NMC).
- Post‑registration experience is helpful but not essential.
- Excellent communication skills and ability to make autonomous clinical decisions.
- Willingness to develop your potential through our pre‑ceptorship programme.
• Permanent role, full‑time 37.5 hours per week.

• Dedicated pre‑ceptorship support for newly qualified nurses.
• Mobile work with laptop and smartphone to maintain patient safety and record keeping on the move.
