Loma Linda University Faculty Medical Group, Department of Surgery at Riverside University Health System Medical Center (RUHSMC), an ACS-COT verified Level I trauma center in Riverside County, CA is seeking a full-time Trauma Care Coordinator and Liaison Advanced Practice Provider (APP) to join our team. The Riverside University Health Systems Medical Center (RUHSMC) is a Riverside county-designated safety net hospital dedicated to the mission of caring for the underserved and to improving the wellbeing of our culturally and socioeconomically diverse region.
RUHS serves nearly 4000 trauma patients annually. The Trauma Care Coordinator and Liaison Advanced Practice Provider (APP) plays a central role in delivering coordinated, multidisciplinary, trauma-informed care to adult trauma survivors following discharge from RUHS. This role focuses on addressing the medical, functional, psychological, and social needs of patients to promote recovery, reduce readmissions, and support successful community reintegration.
Core duties include supporting the team to provide optimal care of the injured patient and leading care navigation, case management, and survivorship activities across inpatient and outpatient settings. The APP will work closely with trauma surgeons, behavioral health, rehabilitation services, social work, and community partners to coordinate care for trauma survivors.
- Support team to provide optimal care of the injured patient
- Care Navigation & Case Management
- Identify patient needs for disposition on a daily basis
- Educate patients, caregivers, and families regarding follow-up
- Identify potential fall-out and communicate with clinic staff to ensure scheduling of appointments
- Coordinate care plans for survivors of significant trauma, including patients with:
- Traumatic Brain Injury (TBI)
- Spinal Cord Injury (SCI)
- Multiple system trauma
- High psychosocial risk factors
- Clinical Care & Survivorship Clinic
- Deliver comprehensive “whole-body” post-trauma care, including:
- Pain and symptom management
- Wound and orthopedic follow-up
- Cognitive and physical function monitoring
- Psychosocial screening
- Medication reconciliation and management
- Pathology should be followed and reported to the patient in a subsequent tele-health appointment (unless there is indication for in-person visit)
- Collaborate with ACS Division Clinic director and TMD to deliver comprehensive outpatient care to the trauma patient
- Participate in Outpatient multidisciplinary initiatives
- Foster growth of the Survivorship program for Trauma outpatient/post-discharge care
- Participate in development of survivorship workflows, inclusion criteria, and clinical protocols
- Collaborate with Trauma Education and Outreach Coordinator to build programming and ensure resource access to patients
- Contribute to trauma survivorship registry, EHR documentation templates, and quality dashboards
- Support data collection, reporting, and participation in trauma committee and quality improvement initiatives
- Assist with program evaluation and refinement
- Licensed Nurse Practitioner or Physician Assistant in the State of California (or eligible)
- Experience in trauma, acute care surgery, critical care, emergency medicine, or related field
- Strong clinical assessment, care coordination, and interdisciplinary collaboration skills
- Demonstrated interest in trauma survivorship, rehabilitation, and trauma-informed care
- Prior experience in trauma services, ICU, or post-acute care coordination
- Familiarity with behavioral health screening tools (PHQ-9, GAD-7, PTSD screening)
- Experience with quality improvement, registry data, or program development
- Bilingual skills and experience working with diverse patient populations preferred
