The Social Services Practitioner III (SSP III – PD) – Per Diem position supports the night coverage at RUHS Medical Center. It plays a key role in patient care coordination and timely discharge planning. The SSP III – PD assists medical and other professional staff in addressing clinical situations and complex cases involving children or adults and their families. Duties include conducting Social Determinants of Health screenings, psychosocial assessments, crisis counseling, discharge planning, and linking patients to community resources and funding sources.

Schedule:

The night shift schedule will be discussed further during the interview.

  • Thursday through Saturday – 12 hour night shift
  • Every other Wednesday – 8 hour night shift
Responsibilities
  • Provide difficult social service casework for complex individual and family problems or social service casework for patients and families in healthcare facilities.
  • Plan, organize, implement, and evaluate all clinical services for pediatric patients and meet program requirements.
  • Participate in in‑service training programs.
  • Provide leadership as a program consultant in the utilization of casework concepts and methodology.
  • Perform other related duties as required.
  • Carry a caseload of difficult social service cases requiring high technical competence.
  • Assess client problems, develop treatment plans, and lead educational courses to community participants.
  • Complete case studies, develop intensive treatment plans, and monitor individual or family functioning.
  • Act in a lead capacity for a small group of Social Services Practitioners or services employees.
  • Act as a casework consultant to staff members without professional training.
  • Provide therapeutic counseling such as marital, family interrelationship, and adoptive parent counseling.
  • Investigate and provide services to clients with physical or emotional wellness issues.
  • Maintain casework records, prepare reports, and correspondences.
  • Frequently conduct field visits, including long distances, to carry out service plans.
  • Assess client family environment for safety risks and manage removal or placement changes when necessary.
  • Maintain up‑to‑date electronic records of all aspects of client case management in a centralized database.
Qualifications
  • Master’s degree in Social Work, Social Welfare, Social or Behavioral Sciences, or related field from an accredited college or university.
  • Strong knowledge of principles and techniques of social casework, public social service agencies, and community resources.
  • Understanding of socioeconomic and cultural factors that promote stable family life.
  • Basic computer skills and ability to input and retrieve data from a computerized system.
  • Ability to establish rapport with clients and families, evaluate personal factors, and provide consultation.
Other Requirements
  • Possession of a valid California Driver’s License may be required.
  • Basic Life Support (BLS) certification from the American Heart Association required upon hire or within six months of hire.
