Service Coordinator
The Service Coordinator is primarily responsible for promoting the belief in cooperative principles and assisting in creating a strong community that adheres to CSI Support & Development policies and federal, state, and local laws. They provide continuous education to empower members to participate in the management of their cooperative, maintain a positive and respectful attitude, and enable members to age in place while remaining independently living in the co‑op.
EssentialFunctions
- Assist and educate members and families on services needed to maintain an independent lifestyle, avoiding unhealthy boundaries.
- Educate members, families, and staff on available community resources.
- Assist members in building informal support networks among themselves and with family members and friends.
- Act as a liaison between community agencies, services providers, and residents.
- Encourage proactive meeting of social, psychological, and physical needs.
- Facilitate meetings of needs when necessary, avoiding unhealthy dependence.
- Use the least restrictive intervention necessary to alleviate a problem.
- Work collaboratively with the members, Council, co‑op liaison, and other committees of the co‑op.
- Complete psychosocial assessments on a voluntary basis to identify member needs.
- Establish relationships with local service providers and monitor the quality and effectiveness of their service delivery.
- Plan and implement educational and group programs.
- Participate in regular training and evaluation of program outcomes.
- Maintain a directory of community services and make it available to members, families, and management.
- Maintain individual files on members and document contacts with members, providers, and families/friends.
- Complete reports and provide copies to appropriate parties in an accurate and timely manner.
- Adhere to company policies, safety regulations, and OSHA requirements.
- Perform other related duties as assigned.
- Engage diversity and difference in practice, applying inclusive strategies and managing personal biases.
- Demonstrate ethical and professional behavior according to NASW Code of Ethics and relevant laws.
- Engage with individuals, families, groups, organizations, and communities using empathy and evidence‑based skills.
- Intervene with individuals, families, groups, organizations, and communities, choosing interventions to achieve practice goals.
- Evaluate practice using appropriate methods, applying findings to improve effectiveness at micro, mezzo, and macro levels.

Requirements Work environmentService Coordinators generally work in an office environment.
Physical demandsRequires sitting at a desk for extended periods and significant time on the phone; must be able to push, pull, lift up to 30 pounds.

RequiredEducation and Experience
- Associate’s degree in Social Work or related field.
- 1‑3+ years of experience working with older adults in a community‑based setting.
- Knowledge of senior community and area resources.
- Good oral and computer skills; proficiency in Microsoft Office.
- Ability to work independently with excellent time management.
- Experience accessing apartments in various conditions.
- Diligence, ownership of tasks, respect, passion, and commitment for working with seniors.
Employees must complete Form I‑9 Employment Eligibility Verification per DHS regulations. Work in the United States requires completion of this form.
