Aboutthe Position
The Behavioral Health Care Manager (BH CM) for Enhanced Care Management (ECM) – Community Supports (CS) program supports the program and advocates by ensuring the quality of clinical service is maintained. The role involves accurate clinical record keeping, guidance to improve student care and program effectiveness, and a patient‑centered approach aimed at optimizing care delivery for high‑need populations.
Responsibilities- Support ECM students with behavioral health conditions, especially those with SMI and/or SUD needs, through brief interventions and behavioral activation strategies.
- Work with a caseload of students with behavioral health needs (majority Tier 1 or 2).
- Utilize brief behavioral health therapeutic interventions to improve students’ self‑management of health.
- Provide brief crisis interventions and warm hand‑offs to local crisis resources.
- Offer student and family education about chronic medical and behavioral health conditions to improve health literacy.
- Deliver formal and informal training for ECM students on treatments and evidence‑based interventions.
- Coordinate medical and behavioral health access with PCPs, specialists, and ancillary services.
- Track medical and behavioral health outcome measures in a patient registry.
- Ensure smooth transitions of care, coordinating with psychiatric hospitals and IEHP transition teams.
- Collaborate with ECM care team members during weekly systematic and ad‑hoc case reviews.
- Help students identify health/wellness goals and incorporate them into the Student’s Care Plan.
- Assist the RN Care Manager with comprehensive health needs assessments.
- Engage students via evidence‑based approaches (e.g., Motivational Interviewing) to promote collaboration and sense of control.
- Provide consultation on clinical tasks for all team members.
- Support the learning and practice of the Teaching and Therapeutic Community model, including faculty training.
- Attend workshops, meetings, and trainings as requested by the supervisor.
- Perform other related duties as assigned.
- Required: Master’s degree in behavioral health, public health, or related human services field.
- Licensed/registered professional in good standing with the California Board of Behavioral Sciences (e.g., ASW, LCSW, LMFT, AMFT, LPCC, APCC).
- Minimum of 5 years clinical experience, with at least 2 years in care management or population health.
- Expert knowledge of clinical practices, care coordination, and patient‑centered care.
- Strong analytical skills to interpret clinical data and translate findings into actionable strategies.
- Willingness to travel locally to support program initiatives, trainings, and stakeholder engagement.
- Certified or licensed as required (BBS licensure or registration, valid driver’s license).
- Preferred: Clinical experience working with individuals recently released from jail or prison; supervisory experience.
- Excellent organizational skills and attention to detail.
- Excellent written and oral communication skills.
- Extensive knowledge of non‑profit organizations.
- Proficient in Microsoft Office Suite or similar software.
- Knowledge of contracting processes and related regulations.
- Ability to multitask, identify problems, and recommend solutions.
- Ability to work constructively with diverse people and varying perspectives.
