Your Role and ResponsibilitiesAs an RN in Home Care, you will:
- Deliver skilled nursing care and case management to patients in their homes.
- Conduct assessments , provide treatments , and educate patients on their health.
- Utilize electronic health records for accurate documentation and adherence to policies.
- Manage a diverse caseload, including admissions, post-hospitalization assessments, recertifications, and discharges.
- Administer IV therapy , perform dressing changes, and provide general skilled nursing care.
- Assist with additional needs to ensure patient safety and appropriate care, such as connecting them with community resources.
To thrive in this role, you should have:
- Education: Graduation from an accredited nursing school.
- Licensure: Current state nursing license and a valid driver's license.
- Experience: Preferably at least one year in a related field.
- Certifications: Current CPR certification.
This is a per diem position , allowing you to provide care on an as-needed basis while following all department policies. Your work will be consistent with our mission, values, and Mercy Service Standards.
