Above and Beyond Monticello is seeking a compassionate and dedicated Home Care Primary Care /
Registered Nurse (RN) to join our home health team. In this role, you’ll provide high-quality, patient-centered care in the comfort of patients’ homes, helping them maintain their health, independence, and dignity.

Coverage area is approximately between Cedar Rapids and Monticello.

Key Responsibilities
  • Conduct comprehensive assessments of patients’ health status and needs
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams
  • Administer medications and treatments, monitor patient responses
  • Educate patients and families on disease management and health maintenance
  • Monitor vital signs and manage chronic conditions
  • Accurately document patient care activities
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Provide emotional support and counseling to patients and families
  • Ensure compliance with healthcare regulations and standards
  • Participate in quality improvement initiatives and ongoing education
What We’re Looking For
  • Graduate of an approved nursing program (BSN preferred).
  • Current RN license in the state of practice.
  • At least 1 year of professional nursing experience (Home health experience is a plus!).
  • Strong computer and technology skills.
  • A valid driver’s license and reliable transportation.
  • Commitment to our mission, ethics, and compliance standards.
