Above and Beyond Monticello is seeking a compassionate and dedicated Home Care Primary Care Registered Nurse (RN) to join our home health team. In this role, you’ll provide high-quality, patient-centered care in the comfort of patients’ homes, helping them maintain their health, independence, and dignity.
Coverage area is approximately between Cedar Rapids and Monticello.

- Conduct comprehensive assessments of patients’ health status and needs.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Administer medications and treatments, monitor patient responses.
- Educate patients and families on disease management and health maintenance.
- Monitor vital signs and manage chronic conditions.
- Accurately document patient care activities.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Provide emotional support and counseling to patients and families.
- Ensure compliance with healthcare regulations and standards.
- Participate in quality improvement initiatives and ongoing education.
- Graduate of an approved nursing program (BSN preferred).
- Current RN license in the state of practice.
- At least 1 year of professional nursing experience (Home health experience is a plus!).
- Strong computer and technology skills.
- A valid driver’s license and reliable transportation.
