Overview

Clinical Pharmacist Specialist – Critical Care Service Line

Tri Health is seeking a dedicated Clinical Pharmacist Specialist to support the Critical Care Service Line at Bethesda North Hospital in Cincinnati, OH. This full-time, day-shift position offers a key role on the interdisciplinary critical care team, delivering direct, patient-centered care to critically ill patients.

Location: Bethesda North Hospital, 10500 Montgomery Road, Cincinnati, OH 45242

Work Hours: Full time, 80 hours bi-weekly;
Day Shift;
Weekend and Holiday rotation

Job Responsibilities
  • Assures pharmacy is an integral part of the health-care delivery system, leading assigned patient service lines, clinical areas, and therapeutic programs; facilitates pharmaceutical care services, direct patient care programs, and medication utilization systems. Designs and implements stewardship activities and restriction/surveillance programs. Tracks and evaluates assigned pharmacy programs for operational, quality, and financial efficiency; benchmarks against local and national best practices and identifies practice-related issues.

    Develops and oversees policies and procedures.
  • Delivers direct patient care and clinical practice, including decentralized and service-based programs; maintains proficiency in decentralized pharmacy services and clinical pharmacy programs. Actively participates in multidisciplinary teams to provide patient-centered care, identifies high-risk patients, implements interventions, and provides discharge medication review, reconciliation, and counseling as appropriate. Participates in the management of medical emergencies.
  • Provides pharmaceutical services throughout the medical center, maintains proficiency in hospital computer systems and medication ordering systems; ensures safe, timely, and appropriate medication therapy. Performs critical patient monitoring, reviews patient profiles/charts to identify, prevent, or mitigate drug-related problems, and communicates effectively with healthcare providers and caregivers. Contributes to education and competency evaluation within the department.
  • Participates in pharmacy operations and medication dispensing; maintains proficiency in central and satellite dispensing pharmacies and specialty pharmacy areas; facilitates specialty medication procurement, ordering, and dispensing procedures (e.g., chemotherapy, parenteral nutrition, controlled substances) as appropriate. Oversees pharmacy operations and technician activities.
  • Supports experiential education and practice advancement; maintains preceptor roles and mentorship skills. Develops student, resident, and staff training experiences/competencies and fosters teaching opportunities. Identifies and implements improvements in the medication-use system to enhance patient safety, maximize therapeutic outcomes, and control costs.
  • Communicates relevant information to other department members and assumes responsibility for professional development, particularly in ongoing clinical education.
Qualifications
  • Bachelor's degree in pharmacy;
    Pharm

    D or Bachelor’s of Science with equivalent decentralized hospital pharmacy experience (3 years) OR accredited PGY-1/PGY-2 residency
  • Pharmacist, Ohio Board of Pharmacy Licensure
  • Clinical mastery in the dedicated area of pharmacy practice consistent with the job posting
  • 1–2 years of clinical pharmacy practice experience consistent with the job posting
  • 1–2 years of clinical pharmacy experience with completion of ASHP-accredited PGY1 residency or Board Certification in a related area of practice
