Position Overview:
We are seeking a dedicated and compassionate LPN Liaison to join our Care Master team to service the Montgomery, AL area. The LPN Liaison will serve as a vital link between our healthcare providers, patients, and their families, ensuring that all parties receive exceptional care and support. This role combines clinical knowledge with strong communication skills to enhance patient outcomes and satisfaction.
Key Responsibilities:- Patient Assessment:
Conduct initial patient assessments and ongoing evaluations to determine care needs, collaborating with the healthcare team to develop personalized care plans. - Communication:
Act as the primary point of contact for patients and families, providing education on care procedures, addressing concerns, and ensuring effective communication between all parties. - Care Coordination:
Facilitate coordination of care among multidisciplinary teams, including physicians, nurses, therapists, and social workers, to ensure seamless service delivery. - Clinical Support:
Provide direct patient care as needed, including administering medications, monitoring vital signs, and assisting with daily living activities. - Documentation:
Maintain accurate and timely records of patient interactions, care plans, and progress notes in compliance with regulatory requirements. - Patient Advocacy:
Advocate for patients’ needs and preferences, ensuring their voices are heard in the decision‑making process regarding their care. - Quality Assurance:
Monitor the quality of care provided, identifying areas for improvement and implementing best practices to enhance service delivery. - Training and
Education:
Provide training and support to new staff, ensuring they understand protocols and best practices in home healthcare.
* On Call is Required
- Current LPN license in Georgia.
- Minimum 2 years of experience in home healthcare or related settings.
- Strong clinical assessment and patient care skills.
- Excellent verbal and written communication skills.
- Ability to work independently and as part of a team.
- Proficient in electronic health records (EHR) and Microsoft Office Suite.
- Strong organizational and time‑management skills.
