(PRN) Speech Therapist (Montgomery, AL) - Pro Health Home Health & Hospice

• Implements and administers Speech Language Pathology services as ordered by physicians.
• Directs and supervises personnel as assigned; participates in care planning with home care team.
• Takes initial history and performs initial evaluations; conducts all skilled procedures.
• Consults with physicians regarding changes in treatment; writes progress reports to physicians.
• Instructs patients and family members in home programs; presents in-service sessions as needed.
• Completes initial evaluation, treatment plan and goals; obtains physician approval; maintains ongoing recertifications (every 60 days if continued).
• Writes progress notes after each visit; performs reevaluations and discharge summaries as applicable.
• Develops and revises the plan of care; assists in determining level of function and revises as necessary.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant changes within agency time frames.
• Utilizes ICD-10 codes appropriately; participates in QAPI activities as assigned.

• Master’s or doctoral degree in Speech Language Pathology, and licensed as a Speech Language Pathologist by the state of practice, or
• Completed 350 clock hours of supervised clinical practicum (or in process), at least nine months of supervised full-time SLP experience, and successfully completed a national examination.
• Currently licensed in the state(s) where practicing.
• Two (2) years’ experience preferred.

Works indoors in the agency office and patient homes; travels to/from patient homes.

• Ability to participate in physical activity; ability to work for extended periods standing and engaging in physical activity.
• Moderate lifting; ability to do extensive bending, lifting and standing on a regular basis.
