AboutThe Role
The RN Care Manager (Director of Nursing) will lead oversight of care delivery and the overall quality of services provided by our company and will participate in the training and supervision of direct care staff. This role works closely with other members of the leadership team to execute Bright Star Care's promise of a higher standard of premium, person-centered care—from companion-level services to skilled nursing care.
Key ResponsibilitiesCare Coordination & Clinical Oversight
- Coordinate care for clients by identifying needs through the assessment process, implementing the plan of care, and managing the supervisory process
- Conduct initial assessments and develop individualized plans of care; reassess and update care plans as client needs change
- Perform skilled nursing services as needed (e.g., wound care, injections, catheter care, infusions, medication setup, etc.)
- Prepare accurate clinical documentation including clinical notes, progress notes, and summaries of care
- Effectively communicate with clients/families, community partners, and other healthcare providers to ensure adequacy of services and continuity of care
- Coordinate with referral sources and partners to support appropriate services and successful outcomes
- Supervise and support RNs, LVNs, and caregivers to ensure standards of care are met
- Provide ongoing assessment of care staff performance, including skills and competency reviews, to ensure high-quality care delivery
- Implement training and in-service programs to promote clinical excellence and caregiver readiness
- Ensure compliance with Bright Star Care standards and promote the organization's quality expectations
- Support adherence to Joint Commission standards and readiness initiatives
- RN Care Manager (Director of Nursing) represents Bright Star Care at local industry events and conferences to network and stay current on trends
- Promote home health referrals in the healthcare community and strengthen relationships with referral partners
RN Care Manager (Director of Nursing)
- Active, unrestricted RN license (required)
- Minimum 2 years clinical nursing experience (home care, primary care, or similar preferred)
- Bachelor's degree in Nursing from an accredited NLN program (preferred)
- Strong leadership ability to mentor, supervise, and guide nurses and caregivers
- Knowledge of state/federal regulations, HIPAA, and confidentiality standards
- Reliable transportation and valid auto insurance
- Clean background/criminal record (required)
- Consistent professionalism, strong judgment, and excellent assessment skills
