Full‑Time Minimum Data Set Nurse (MDS‑LVN/LPN)

Salary Range: $92k–$97k per year

• Assist in the completion of the Minimum Data Set (MDS) assessment for all residents.
• Coordinate the MDS process, ensuring accurate and timely submission in accordance with state and federal regulations.
• Collaborate with interdisciplinary teams to gather necessary data and support the resident’s care‑planning process.
• Maintain accuracy and integrity of resident information by ensuring all data is entered correctly.
• Monitor resident status and changes that may require updates to MDS assessments.
• Assist with compliance with regulatory requirements governing MDS, including Medicare participation.

• 1–3 years of MDS experience.
• Nursing degree from an accredited college.
• Current and unencumbered LVN/LPN license in the state.
• Knowledge of clinical standards of practice, regulations, and reimbursement governing long‑term care and Medicare participation.
• Proficiency in nursing practices, procedures, terminology, laws, regulations, and guidelines related to long‑term care.

• Experience providing care to multiple residents.
• Experience in a Skilled Nursing environment.
• Experience with PDPM (Patient‑Driven Payment Model).
