In this role, your primary responsibilities include developing, implementing, and updating individualized treatment plans in collaboration with supervising therapists and the broader rehabilitation team. You’ll conduct patient evaluations, measure outcomes, adjust interventions to maximize functional gains, document progress for compliance and reimbursement, educate patients and families on home programs and safety strategies, and participate in team conferences to align goals with overall discharge plans.
You’ll facilitate therapeutic activities that foster strength, coordination, cognition, and endurance, applying evidence‑based modalities to optimize participation in daily tasks and meaningful occupations. You’ll contribute to program development by identifying opportunities for process improvements, sharing best practices with colleagues, and supporting new staff through onboarding and coaching.
This assignment provides 38 guaranteed hours per week, with a structured schedule designed to support consistent patient contact and meaningful outcomes. The competitive weekly pay range is $1,298 to $1,331, and compensation packages will be confirmed during the hiring process. Start date is 01/26/2026, and the duration is weeks, with extension opportunities depending on performance, site needs, and patient volume.
