Position Overview
We are seeking compassionate and driven Lead Care Managers to support members enrolled in the CalAIM Enhanced Care Management (ECM) program. Lead Care Managers are assigned to enrolled members to coordinate services, provide resource navigation, and address social determinants of health through a whole-person, person-centered approach.
This role is highly community-based, with approximately 90% of work conducted in members’ homes
, as well as hospitals, shelters, and clinics. Telehealth services may be provided when appropriate based on member preference or scheduling needs.
- Provide community-based case management, care coordination, and member support for individuals enrolled in the CalAIM Enhanced Care Management (ECM) program
- Conduct outreach and engagement activities, including tabling and community events, and collaborate with team members to engage hard-to-reach or socially isolated members
- Coordinate housing navigation services, including housing assessments, individualized housing support plans, security deposit assistance, utility setup, and education on landlord/tenant rights
- Conduct health and safety home visits and unit habitability inspections in members’ homes, as well as visits to hospitals, shelters, and clinics; provide telehealth services as appropriate
- Support members in accessing community resources, benefits advocacy, and public assistance, including obtaining identification and documentation and supporting SSI eligibility and application processes
- Enroll new members into the CalAIM program and maintain accurate, compliant documentation and housing charts in accordance with agency and contractual requirements
- Educate and guide members in understanding, navigating, and utilizing their insurance benefits to address social determinants of health and complex care needs
- Medical Assistant (MA), Certified Nursing Assistant (CNA),
or Community Health Worker (CHW) background and experience - Reside within the county and have the ability to travel locally
- Valid driver’s license, clean driving record, active car insurance, and reliable transportation
- Bilingual in Spanish (preferred, not required)
- Field-based role with frequent home, community, hospital, shelter, and clinic visits
- Combination of in-person services and telehealth, depending on member needs
